Log in to UltraLooper and go to your dashboard. Click the grid icon (All apps) at the bottom of the left sidebar to open the full app launcher, then choose UltraLooper Forms. UltraLooper Calendar, UltraLooper Sheets, UltraLooper Slides, UltraLooper Forms, and UltraLooper Approve are listed under Docs & planning in that menu.
After opening UltraLooper Forms from the All apps menu, use Form builder and Responses from the UltraLooper Forms area (or /ultraform/builder when logged in).
